Nursing Care in the home when discharged from Hospital

 

HELP Vega Baja is pleased to announce the introduction of a pilot scheme for members who require a caring service at home following discharge from hospital

 

The conditions for eligibility:

 

  1. Current membership of HELP for at least six months
  2. Discharge from hospital following in-patient care
  3. An assessed need for home support due either to:

 

·         An inability to undertake daily living tasks and/or personal care,

·         Not having an available carer at home,

·         The carer at home being assessed as not capable of providing the necessary caring service at home.

 

How to make the application

 

The HELP Association must be contacted as soon as a discharge date and time has been agreed. The San Miguel office should be contacted in the first instant or the emergency help line outside of normal office hours.

 

The HELP Association will then notify the Nursing Care Provider who will arrange to carry out an assessment visit.  The assessment visit will establish the level of care and support needed, and provide a 24 hour emergency number

 

The HELP Association will fund the agreed service for up to 7 days following discharge from hospital. At the end of the agreed period the funding from HELP will cease and any further care will be at the members own expense.

 

The intention is to supplement the services available from the Spanish health services and not replace them.

Emergency Line

The volunteers who man our 24 hour emergency line both day and night respond to a variety of calls. Typical enquiries are ‘what to do when a death occurs’ – They are also asked what to do when someone is taken ill in the night, or has been robbed. Other calls include enquiries about loss of passports and for locksmiths. To obtain the numbers of the volunteers on duty when the office is closed just call  966 723 733.
